I hope this letter finds you and your families well. As you may be aware, the Center for Disease Control and New York State have approved administration of the Pfizer vaccine to children ages 12 and older. In light of this, the Chautauqua County Health Department will conduct a voluntary and free vaccine clinic to all eligible students and their parents/guardians on Friday, May 21 at 9 a.m. in our high school gym.
In the links bellow, please find the registration, consent form and pre-screening form for your child. If you are interested in having your child vaccinated against COVID-19, please complete the forms and return it to the school nurse as soon as possible. Again, it is the Pfizer vaccine, so two doses will be required. The second dose will be given three weeks later, on June 11. You will receive information regarding your second dose appointment when you receive your first vaccine.
A parent or legal guardian may accompany their child under the age of 17 to their vaccine appointment. If you want your child to receive the vaccine but the forms are not complete, please be available by phone between the hours of 9 and 10 a.m. on Friday.
We request that you please return your child’s consent form and note any additional eligible household members you wish to have vaccinated in advance so we can coordinate with the county and have enough vaccine doses on hand that day.
